Just had the ICP replaced 2 weeks ago along with #5 injector and they flashed me for the first time. Started doing things it had never done before once I got it back. The tech said it was probably wire chaffing, never had it done under the recall. Runs fine 99% of the time, when not running right it idles up and down from 750 RPM to 1100 RPM. Surges, power cuts out etc.
Hopefully they can get it fixed. It was problem free until 62,000 miles.
well they might not have cleared the code, and lord knows you would NOT
be the 1st that had a part replaced that didnt actually NEED fixin'
your surging/cutting in and out could be the wire harness chafing...
there where several different locations were wires wore/rubbed thru and
grounded out either against each other or against metal/bolts etc.....the
problem is a bit troublsome since they were intermittent....some trucks
were dying in the middle of the road/interstate !
there was a FICM chaf as well as an ICP chafing issue, you might wanna
review the links & summary below...might even show it to the dealer if
youre not getting satisfactory resolution (photos are included)....and
the wooden dowel/stick trick might help find an intermittent issue.

In this article we highlight some issues concerning the injector
harness on the F-Super Duty 6.0L Power Stroke diesel engine that
has been troublesome to owners and technicians. This is not intended
to be a complete showing of commonly known areas of harness chafing.
Some 2003-2005 6.0L PSD engines may exhibit the following conditions: no
crank/no start, cranks/no start, bucking and stalling. Though we are
focusing on the injector harness here, the main engine sensor harness
is equally susceptible to chafing and crushing.

Safety Recall 05S34
2004 F-Series F250 though F550 Super Duty trucks and Excursion sport
utility vehicles built from September 29, 2003 though November 30, 2003
will have the Fuel Injection Control Module (FICM) wire harness inspected
for wire chafing and exposure. If no exposure is found, a repair kit is installed
on the harness to prevent chafing and damage. A harness that is damaged
and has exposed wiring that causes symptoms like stalling, will be replaced.
A road test will also be performed to verify any repairs and identify additional
driveability concerns that may exist.

actually i took the module off , and have
been running with the factory flash for the last two weeks since i
disconnected the egr just to see. no my truck is not a california truck,
but i at least expected a code even though i didnt see a light. you may
be on to something, but again, i just got my sct today and this is the
first time i checked for a code. the only other programmer i had run
was the edge module and it's been in the trash for a while now.
my other truck was all done by roland at RBP and they used a sct type of downloader on it, this truck has never had the stock ecm screwed with.
interesting...i would have expected codes, mine generated 404 & 405
when i disconnected it...i was also having an egr failure at the time too.
i have run w/ the egr disconnected and gotten 404s, but when the
egr was plugged back in the codes were eventually cleared...i just thought
it was a fluke though.
hummmm now im gonna have to do some playing to see how often
the codes are gen'd and when they clear themselves...
