Not true. usually it's ONLY one or the other. If you have oil in your coolant (in most diesel engines) the problem is most commonly the oil cooler, because the oil is higher pressure than the water. If you have coolant in your oil, it is usually a block/ sleeve/ head problem, water leaking directly into the crankcase.If you have oil in your water, you'll probably have water in your oil, and they can detect that.
