I think theres to much thinking here........LOL, but it's all good. The throttle works the same. It's still sitting on the manifold, but the throttle is sitting on the blower so to speek. The blower could be gear drive off the fly wheel or off the HP fuel pump under the intake. so no drive belt needed. this would take care of the EGR too.

I'm no expert, but looking at the photo it looks like the ribbed part of the belt is traveling against the tension pulley. Shouldn't the flat side of the belt be riding against the tension pulley?

you are correct in your thinking, however often with HI-PO applications, sacrifices are made in the longevity and noise department, thus the ribs being on the pulley.
