Reason I mentioned it Don is that in the 7.3 realm his symptoms are classic of the o-rings failing on an IPR. Once they get worn they allow the injection pressure to leak by the IPR valve. When the valve is cold and the oil is cold you dont experience the leak by.

IPR Rebuild on a 7.3

Just my dos centavos:2c from a guy that is new :newhere in the 6.0 but trying to learn and help at the same time.

mike it makes good sense and certainly could be a cause...my thought
is that his engine does START when cold....so i tend to DOUBT its
an HPOP leak <down>.

HPOP leak downs cause problems starting <cold>....

in the 05-07s owners have reported increasingly longer cranking times...
(assuming the xtra cranking is needed to build the 500psi
minimum required to start)...once the pressure builds (via cranking) on
these motors they run...

certainly the IPR & ICP need to be checked...and god knows even on an
03 they are gonna be easier to replace than tearing stuff off the top just
to get to the HPOP

there are no codes, and everything on the MODIS reads fine until you shut the truck down, the when you go to start it. the ICP readings are to low
