With 4000# that hitch should have been fine. Rough roads? Shock loading with a tow strap? Even with the spacers out of alignment I can't see why it would have broke like that..

i can weld them but i have new brackets to replace them with. i bought the truck about a year ago so this was probably done before i got it. i have not hauled more then 3000# since i have had it.

the hitch I have is an Eckhart ( do not know if they are available nationally or just a local brand...los angeles) but they make theirs to weld on only. also, if memory serves they also do not have the tabs as a part of the vertical simply bent up to the frame (the bend being where the stress relief is on yours that eventually cracked).
ive had them do a couple hitches for me and they always have a pile of other brands sitting that are taken off, lots of which are the bent style many of which are cracked.

Spacers are normal and come with the hitch. I would upgrade to a class V and get it over with.
Glad you caught it before it fell off while loaded!
