Glow plugs will cause smoke on a cold start. It will run crappy until it warms up. Diesels don't like to be run cold, they don't make a whole lot of power when cold. Let it warm up before you call on it to do some hard work.

sticking/varnished fuel injectors will cause a rough idle and then smooth up when they warm up.the early trucks ford designed a reflash to help improve the cold start.the rough idle statagy was designed for trucks that missed at 60 degrees and below .

I have an 05 F-350 that was recently updated so the injectors all pulse after the truck is shut down, draining the fuel out of them. You can hear a real quick ticking sound for a split second if you pay attention. This supposedly helps with cold starts.

It's supposed to be a factory reprogram from Ford.
