Found it.
While I had the trans out I was looking at the driveshaft and noticed that one side of it wasn't rusty. I put it back in with that clean side lined up as best as possible and the vibration is pretty much gone.
At some point someone installed it clocked about 90 degrees off (looking at the lines I drew when I removed it).

It's probably not exactly right, but it's a he11 of a lot better now.

I think I may have found a similar problem on my truck as well. I was taking the running boards off my truck and the sun was out and really hot....so I did all the work from underneath my truck. Laying there...resting....I saw a fixed bearing assy:dunno that holds the drive shaft right before it turns down headin to the differential. It looks like a seal or something is busted around the top but I cant get my BIG head up there to tell for sure. There is some wiggle room when I shake it up and down right there.

Glad to hear you got your problem fixed. I plan on when I get about $20k saved up....I'm gonna have that truck gone through for EVERYTHING. A guy can dream....right???
