Does anyone have instructions for installing a Banks BHA on the stock turbo?

Remove the old wastegate actuator. Bolt BBHA on in place of the old one. Set so that it JUST drops onto the wastegate arm, then take it off, and turn it In/shorter by six full turns. Then stretch it back over onto the wastegate. Easiest to do this using a bit of compressed air on the actuator nipple... arm will shoot right over and then you drop it down.
