OK then, shouldn't the bed be lower than the cab?
I replaced my cab bushings four years ago. My bed alignment was way worse than your photo.

Im pretty sure the bed is always lower than the cab
......but i guess its worth looking into it had a 5th wheel in it at one time but it was never used it pulled a 20 sumthin foot bumber hitch enclosed trailer for a roofing company....so im sure my mounts are goin bad.....but i need to Rebuild my IPR and get a new Fuel pump and tires first
