Wheel separation and stud shear

Had my wheel come off and all studs sheared off while pulling my fifth wheel. 270 miles after having tires rotated. Picture of hub, outside of wheel and inside of wheel. Can any experts chime in and tell me why my wheel came off? It was scary Af.

The lug nuts were not torqued properly (they were loose). I use a long wrench and lean on it.

“Retorque lug nuts after 50 miles” is the disclaimer on every receipt I get for any work requiring a wheel to be removed.
